What it is:
St. John's Wort is an aromatic, perennial herb which blooms yellow flowers around the time celebrated as John the Baptist's birthday, June 24th, hence the name. The flowering tops are the part of the plant (Hypericum perforatum) that is used.
St. John's Wort has a popular history and use today for enhancing mood and emotional well-being. The mood-elevating effects of St. John's Wort were originally thought to be due solely to the active component, Hypericin, but Hypericin does not act alone. As with many herbal medicines, St. John's Wort relies on the complex interplay of many active constituents (e.g. xanthones and flavonoids) for its mood enhancement. St. John's Wort may also block the receptors that bind serotonin.
The recommended dosage for St. John's Wort is 300 mg of the dried leaf and flower extract standardized to at least 0.3% hypericin, 3 times a day. Since St. John's Wort has a very long half-life of about 25 hours, it can actually be dosed all at once for the same effect, and to increase compliance. People who tend to be sensitive to the sun (such as fair-skinned individuals), should be cautioned when taking St. John's Wort, or those individuals taking large doses of the herb should also be cautioned. In addition, St. John's Wort may have additive effects with SSRI's and other antidepressants, and should only be used at the same time if the person is under physicians care. St. John's Wort is also contraindicated in pregnancy because it may stimulate uterine contractions. More recently, it has been discovered that SJW may affect the Cytochrome P450 system, which is responsible for metabolizing various drugs or medications. Therefore, St. John's Wort may affect the metabolism of certain drugs such as: WARFARIN, DIGOXIN, THEOPHYLLINE, CYCLOSPORINE AND ORAL CONTRACEPTIVES. PATIENTS ON ANY MEDICATION SHOULD CHECK WITH THEIR PHYSICIAN OR HEALTH CARE PROFESSIONAL, BEFORE TAKING ST. JOHN'S WORT. Also, individuals taking St. John's Wort should use caution in consuming several foods that contain tyramine that also influence the P450 system such as broccoli, red wine, dried cheeses, pickled herring, to name a few. St. John's Wort may be recommended as part of a daily regimen for Mood/Emotional Well-Being.
